Shock Loss and Shedding After a Hair Transplant: Why It's Normal
Around weeks two to four, transplanted hairs fall out. It looks alarming and it is completely expected. Here is what is happening and when growth returns.
Few stages of hair transplant recovery cause more anxiety than the shedding phase. You had a procedure, you saw new hair, and now it is falling out. Rest assured: this is not failure. It is the plan working exactly as biology intends.
What is actually happening
A transplanted graft carries the follicle — the living factory — plus the hair shaft it produced at the donor site. When the graft is relocated, the old shaft goes into a resting phase and is released over the following weeks. What falls out is the temporary shaft, not the follicle. The factory stays; only the product is cleared for a new production cycle.
New growth typically becomes visible around months three to four and continues thickening well into the second year.
Shedding vs shock loss
Shedding of transplanted hairs in the first six weeks is universal and temporary. Shock loss refers to native hairs near the transplanted zone temporarily thinning from the surgical disturbance. It is also usually temporary, but it is the phenomenon patients find most unsettling — suddenly existing hair looks sparser right after you paid to make it denser.
- Expected: transplanted shafts fall out between weeks 2 and 6.
- Expected: some native hairs near the work zone thin temporarily.
- Not expected: increasing pain, spreading redness, pus, or fever — these warrant contacting your clinic immediately.
How to get through the phase
Follow your washing instructions precisely — the first washes protect the grafts, and gentle technique matters more than any product. Resist the urge to judge your result during shedding; nothing meaningful can be assessed before month three. Take photos in consistent lighting every two weeks. When new growth arrives, those photos will show you progress you cannot see day to day.
If you are worried at any point, message your clinic. A two-minute answer from the team that performed your surgery beats weeks of forum anxiety.
